OUR SUPPORT AT HOME SERVICES

We provide a range of allied health services for people receiving Support at Home packages, helping older adults maintain independence, health and quality of life while living in their own homes.

Our clinicians deliver services directly in the home and community environment. This allows us to understand how you move through your daily routines and identify practical ways to support safety, mobility and wellbeing.

Our mobile Support at Home services include physiotherapy, occupational therapy, exercise physiology, dietetics, speech pathology, remedial massage therapy, and allied health assistant support.

COLLABORATIVE SAH SUPPORT

Many people benefit from support across multiple allied health disciplines, and our clinicians work collaboratively to ensure care is coordinated and aligned with each person’s needs. We also work closely with families, carers and case managers to ensure support is delivered effectively.

Our goal is to help people remain safe, comfortable and independent at home for as long as possible, while supporting overall health and wellbeing.

We provide our services to people everywhere from the Fraser Coast and Sunshine Coast to Brisbane and the Gold Coast.

FREQUENTLY ASKED QUESTIONS ABOUT NDIS

How do I know which allied health service I need?

Every person’s journey is different. During your initial discussion with us, we look at your Support at Home plan and your personal goals, whether that’s moving more easily, improving your diet, or managing pain. We then suggest which of our clinicians, such as a physiotherapist or occupational therapist, might be best suited to help you meet those goals.

While a GP referral is always welcome and helps our clinicians understand your medical history, you can often access these services directly through your Support at Home package. If you are unsure that you have the right ‘approval’ in your plan, our team can assist you in checking this.

It simply means our team talks to each other. For example, if our Dietitian is helping you with nutrition, they might work with our Speech Pathologist to ensure the meal plan is safe for your swallowing needs. This ensures everyone is working toward the same goal: keeping you safe and healthy at home.

Under the Support at Home program, you have the right to choose your providers. We work alongside your package manager to ensure the services we provide align with the goals identified in your My Aged Care assessment.
