Referral Submitted

Referrers can complete our simple online referral form. A confirmation message will appear once the referral has been successfully received.

Our Team Reviews the Referral

Our admin team receives the referral and sends a follow-up message to acknowledge it. The referral is categorised by service type (e.g., NDIS, Support at Home, or private patient) to direct it to the right team.

Matching the Right Clinician

Our team reviews the referral in more detail, including the services required, location, and clinical needs. We match each client with the most appropriate clinician based on their expertise and availability. If we need more information, our team will contact the referrer.

Appointment Confirmation

Once a clinician is assigned, the appointment is scheduled and confirmed with the client and referrer. We send reminders before the appointment to ensure everyone is prepared.

Ongoing Communication and Support

Our clinicians, client care coordinators, and admin team manage communication with referrers, clients, and families to ensure the process is transparent and supportive. We understand that good communication and reliable service are essential to building trust with both the people we support and the professionals who refer to us.
