If your feedback or complaint relates to the National Disability Insurance Scheme (NDIS), you can also access the official NDIS complaints process using the downloadable form below. This option allows participants, families, carers and advocates to raise concerns directly with the NDIS or the NDIS Quality and Safeguards Commission if required.

TRS is committed to ensuring all feedback and complaints are handled respectfully, confidentially and fairly. Providing feedback or making a complaint will not impact the services or support you receive from our team.
