WELCOME TO THE FRASER COAST

Mobile Allied Health Services Fraser Coast

We provide mobile allied health services across the Fraser Coast, including Hervey Bay, Maryborough and surrounding communities, helping you stay healthy, independent and supported in your own home.

Our team support:

  • NDIS participants,
  • Support at Home (aged care) clients
  • Private patients

The Fraser Coast is home to a growing population of older adults, families and people living with disability and our clinicians deliver services directly in the home and community environment. This allows us to understand how you move through your daily routines and provide care that is practical, personalised and focused on real-life outcomes.

Supporting NDIS, Support at Home and Private Clients

Our Fraser Coast team supports people across a wide range of health needs and life stages.

Our team regularly works with:

  • NDIS participants living with disability
  • Older adults receiving Support at Home packages
  • Private patients seeking community-based allied health care

Our services are tailored to your goals, health conditions and support environment. Whether the goal is improving mobility, supporting rehabilitation, managing health conditions or maintaining independence, our team focus on you to provide practical outcomes that make a difference in your everyday life.

A Collaborative Approach to Care

Many people receiving allied health support benefit from input across multiple disciplines. Our Fraser Coast clinicians work closely together to ensure care is coordinated and aligned with your needs.

For example, physiotherapists may support strength and mobility, occupational therapists can assist with home safety and daily activities, and dietitians or speech pathologists may support nutrition and swallowing safety.

At Team Rehab Solutions, we pride ourselves on our multi-disciplinary teams and our collaborative approach helps deliver safer and more effective care.

Working With Referrers and Families

We value the strong relationships we have built with referrers across the Fraser Coast. Our dedicated clinicians, client care coordinators and administration teams work closely with support coordinators, case managers, general practitioners and other health professionals to ensure you receive the right support at the right time.

We accept referrals from:

  • Support coordinators
  • Case managers
  • Healthcare professionals
  • Families and carers
  • Private clients

Our Fraser Coast team is ready to support people with allied health care delivered directly in their homes and communities. Click to submit a referral below or to call your friendly Fraser Coast team.
