OUR NDIS SERVICES

We provide a range of allied health services for National Disability Insurance Scheme (NDIS) participants, supporting people living with disabilities to improve independence, health and participation in everyday life.

Our clinicians work directly with participants in their homes and communities. By delivering services in people's everyday environments, we better understand their routines, challenges, and goals, and we can provide practical support that makes a meaningful difference.

Our mobile NDIS services include physiotherapy, occupational therapy, podiatry, exercise physiology and dietetics.

COLLABORATIVE NDIS SUPPORT

We work closely with participants, families, support coordinators and care teams to ensure services align with NDIS plans and individual goals.

Our clinicians and client care coordinators place a strong emphasis on clear communication, reliable service and high-quality reporting to support ongoing funding and plan reviews.

Through collaborative, person-centred care, our goal is to help participants build confidence, maintain independence and participate more fully in everyday life.

We provide our services to people everywhere from the Fraser Coast and Sunshine Coast to Brisbane and the Gold Coast.

FREQUENTLY ASKED QUESTIONS ABOUT NDIS

Can I have therapy at home or in the community?

Yes, that is what we specialise in! Our mobile service means we come to you. Whether it’s at your home, your workplace, or a local park, we provide support in the places where you spend your time so that the skills you learn are practical for your everyday life.

Yes, after the occupational therapist’s initial assessment, an initial summary report will be completed. This report includes all key information discussed during the assessment and provides recommendations for any occupational therapy input.

It will be provided to you and your support coordinator to help understand the recommended hours of occupational therapy and outline what you and your occupational therapist will be working on together.

This report typically takes 2-3 hours to complete.

Our service agreement is a written agreement between you (or a family member or friend you represent) and your allied health professional at Team Rehab Solutions. It outlines the supports and services to be provided, including associated costs and service schedules.

The agreement also sets out how concerns will be managed, as well as the responsibilities of both the participant and the allied health professional. It includes details on cancellation policies, how the agreement will be reviewed, and how either party may modify or end the agreement.
